4세 때 supracondylar Fx로 local에서 CR & cast immobiliztion한 후 cubitus varus deformity 발견되어 외래 F/U함.
Deformity progression은 뚜렷하지 않음.
Elbow & forearm ROM full
pain on exercise(-)
preop carrying angel var 17도 / val 3도
--> 20도 closed wedge osteotomy 시행하기로 함.
op) corrective closed wedge osteotomy, distal humerus, Lt
(supination 10도 correction,medialization 5mm)
distal humerus posteromedial approach로 ulnar n. exploration하여 identification하고 entrapment 될 수 있는 부위에서 decompression함. triceps medial border를 따라 distal humerus의 supracondylar crest 에서 humerus의 posterior periosteum위로 triceps의 lateral border까지 muscle detach후 triceps를 medial로 retract하여 lateral closed wedge osteotomy 20도 시행하고 lateral hump 교정 위하여 distal fragment를 medial로 5mm translation시행하고 supination 10도 교정하여 bone contiguity를 맞춤. 2개의 smooth S-pin과 threaded S-pin으로 osteotomy site에 internal fixation시행함. closed wedge osteotomy하여 얻어진 bone을 lateral portion의 step off 부위에 graft하여 remodeling 될 것을 기대함.

5-10세경(8-10세경)이 bone handling과 remodelling에 적절한 시기임
